This form is to be used for ALL accidents, or near misses, whether an injury occurred or not.
A member of staff, or responsible hirer, should complete this form. The person making the report should use their best judgement as to how many witness statements are required, based on the severity of the accident or uncertainty as to the circumstances involved.
In completing the report, please be proactive in ensuring you have collected essential information including the casualty’s name and contact details.
Please take and attach sufficient relevant photos. These should include photos of injuries sustained, the area where the incident occurred, or anything that may have contributed to the incident.

Please complete within 24 hours of the incident. If the incident caused, or could have caused, serious injury or property damage, please inform Belconnen Arts Centre on 02 6173 3300 immediately.
